Allambi Care’s Victorian Residential team have been focusing on exploring options to transition our young people into more appropriate care models that match their goals and case planning directions. They've been doing this by utilising Targeted Care Packages (TCP), which is a flexible funding packages referral program aimed at providing financial funding to have young people return to the family home, reunification with extended family or transition into independent living.

Alongside with submitting the TCP, most young people are also being referred to the Better Futures program. This program explores their important people, care team members, their health and wellbeing, current education or employment and their community or cultural connections to develop and support the young person into the next phase. Currently our team are working on transitioning five of our young people into new semi-independent units and supporting the transition of two siblings into a family reunification house through Targeted Care Packages (TCP) and the Better Futures programs via the Department of Families, Fairness and Housing. 

We have been successful in the reunification of two Aboriginal young people in our care. We are supporting them to transition into the care of their grandmother. This has taken an enormous amount of effort and commitment from our team over the past 18 months. The work to achieve this reunification has been a true reflection of Allambi Care’s multi-disciplinary approach, with our Victorian Residential team working in collaboration with our Cultural Support Officers, our Family Search and Engagement team, our Clinical team, and our Foster Care Team.

Another outstanding outcome from our service is that one of our young people is transitioning into one of the new semi-independent units. Initially entering care with Allambi Care on a community corrections order with 2:1 staffing required, this young person has been able to maintain a stable placement for the past eight months whilst showing huge personal growth, due to consistent support, planning and care from the team. Being accepted to take the next phase of the SIL program is a massive step in this young person’s life and one we are so proud to see happening!

A lot of wins and challenges have been learnt along this journey. We are extremely proud of the enormous effort and commitment that our Victoria team has shown to achieve such amazing outcomes for all the young people we have transitioning from residential care.
